Aim. To reveal and evaluate predictors of infectious complications in the postoperative period after pancreatoduodenectomy.
 Materials methods. An analysis was made 790 articles that dealt with subject Results. Seventy five met inclusion criteria. Studies small samples (less than 20 patients) were not considered. Based on multifactorial nature problem preventing period, results review are divided into several sections. PBD is highly effective obstructive jaundice syndrome at preoperative stage, but associated risk bile infection period. These circumstances should be taken account when determining indications for PBD, choosing method decompression, cases where stenting used, attention also paid to choice material. Bacteriobilia an independent predictor surgical site infections (SSI) PDR.
 Conclusion. Changing approach antibiotic prophylaxis based sensitivity microflora can lead a decrease incidence SSI, reduction duration hospitalization, number long-term therapy, rate spread resistance financial costs institution completed case medical care.
